Membership
Per M.S. 256B.0625, Subd. 13., the formulary committee will now be comprised of four licensed physicians, at least three licensed pharmacists, and a consumer representative. One of the physicians must be actively engaged in treating mental illness. The pharmacists and physicians can not work for Human Services and must be actively practicing. Members are selected after consulting with professional organizations.

Entity Powers and Duties, Activity Summary
Per the new M.S. 256B0625, Subd. 13., the formulary committee is designated by the commissioner to carry out duties as described in M.S. 256B.0625, subds 13-13G. These include reviewing the over-the-counter drugs and making recommendations about prior authorization. The committee also still establishes the list of drugs excluded from mandatory generic substitution under MS 151.21.
